Members that participate in the vaccine buying group, should have received the first rebate check of 2021 in April which reflects vaccine purchases between July 1-December 31, 2021. CPP members have been taking advantage of receiving vaccine rebate dollars via automatic bank transfer.
If you would prefer to receive your rebate via electronic deposit, please submit this ACH authorization form to CPP at cpp@nationwidechildrens.org or fax it to 614-722-2149. If you already submitted this form to CPP, you do not need to complete it again.
